Visiting Services
Podiatrist
Rachel Haack (RN) has been providing Podiatry clinics from Cobar Primary Health Care Centre for the past 4 years on a contract basis with NSW Outback Division of General Practice (ODGP). Currently Rachel is funded under the Chronic Disease Management and Prevention Program 3 days per month. Patients must meet the criteria under the program to be eligible for services. Referral & current management plan (GPMP) required.
Dietitian
Laura Bate and Speech Pathologist Olivia Hicks are employed with ODGP and also providing services under the Chronic Disease Management and Prevention Program 2 days per fortnight. Patients must meet the criteria under the program to be eligible for services. Referral U& current management plan (GPMP) required.
Aboriginal Health Worker
Dawn Howlett (EN) is available Monday- Wednesday for Aboriginal and Torres Strait Islander health assessments, and Closing the Gap/Indigenous Health assisting the GP’s with care planning and Management of Chronic Disease.
Cardiologist
Prof Roger Chatoor visits Cobar one Friday per month to review Cardiology patients and is happy to see newly referred patients at the Health Centre. There is a fee for this service-please enquire at reception.
Gynaecologist/Obstetrician
Dr Diane Avery visits the Practice on the third Thursday of the month.
A referral is required for her & there is a fee for service. Appointments can be obtained by phoning Dr Avery’s Dubbo rooms.
Physiotherapists
Meredith Schwilk from Orange runs a private clinic from rooms at the practice Wednesday to Friday fortnightly. Standard bookings can be made at the practice, Workers Compensation consultations must phone 02 6361 7877.
Jackie Haines visits Cobar 1 week every 6 weeks & provides private physiotherapy services to the nursing home & the general public. Bookings can be made at the practice. Both these are fee for service- enquire at reception.
Osteopath
Kylie Read hails from Newcastle and provides a private clinic from the rooms at the practice. Kylie is here Monday to Friday on a 6-weekly basis. No referral necessary. Bookings can be made at the practice- fee for service, enquire at reception.
Naturopath
Kelvyn McCarroll also from Newcastle is available for private consultations on Thursday & Friday’s every 6 weeks. As a Naturopath Kelvyn can help you consider your whole health picture, including your diet, lifestyle, energy levels, sleep quality, stress levels and medical conditions. She can work with you to make positive changes to your health and wellbeing. No referral required. Bookings can be made at the practice-fee for service, enquire at reception.
Social and Community Coordinator
Cobar Primary Health Care Centre in partnership with Western NSW Local Health District has identified the need for a Social and Community Coordinator. The position is to support patients to stay happy, healthy, and in their own homes. Some of the services include; referrals to the Aged Care Assessment Team (ACAT), access to Home Care services, Community Transport, organise personal alerts, carer supports, referrals to Mental Health Services and supporting families with referrals to Royal Far West in Manly. You can self-refer or ask your GP to see the Social and Community Coordinator.
