Bourke Pathways Care Navigation Program

The NSW Outback Division of General Practice (ODGP) is excited to implement the new Bourke Pathways Care Navigation Program, designed to support residents of Bourke Shire in accessing existing health, community, and social services. The Bourke Pathways Care Navigation Program is funded by nib Foundation and the aim of this pilot program is to improve health outcomes and also support self-management and health literacy. Whether it's assistance with referrals, paperwork, or simply knowing where to turn for help, the program is here to ensure that the community receives the care and support it needs.

Program Overview: The Bourke Pathways Care Navigation Program, now operational at ODGP’s main street office in Bourke, is a free service for eligible local residents who may be experiencing difficulty navigating the complex landscape of health and social services. With many community members unaware of available services or unsure how to access them, this new initiative will bridge the gap by guiding individuals through the process, offering personalised support and referrals.

What We Offer:
•    Service Assistance: Help with understanding and connecting to local services, including health, social support, housing, and more.
•    Referrals & Paperwork Support: Assistance with completing forms, referrals to appropriate agencies, and navigating administrative processes.
•    Personalised Guidance: One-on-one care navigation, tailored to the unique needs of each client, ensuring that no one is left behind.

The program was developed in response to community feedback and aligns with ODGP and the nib Foundation’s shared commitment to improving access to services for Bourke residents, especially those who face challenges in accessing care.

Why It Matters: Many people in rural communities like Bourke struggle to find the right services or feel overwhelmed by the paperwork involved. The Bourke Pathways Care Navigation Program is dedicated to simplifying these processes and ensuring that all eligible clients can get the help they need when they need it. This initiative will not duplicate any existing services, and instead work alongside local residents, service providers and community groups to create a new support system.
