Renewal Process for Broadband for Health 2006 - 2007
This is information for those practices that successfully applied for the BFH in 2005-2006 and have not reapplied for 2006-2007. Unless you complete the following you will not receive the second year of subsidy, nor the $1000 incentive payment for filling out the Security Awareness and Conformance Report. What to do 1. Contact your current Internet Provider and request a ¿Statement of Supply¿. This document contains the details of your contract and will be required when filling out your application form. 2. Complete the Broadband For Health Subsidy Application 3. Preferably with the assistance of an IT professional, fill in the Security Awareness and Conformance Report. This document serves as an audit of your practice¿s security status. 4. Fax the Incentive Claim form, Security Awareness and Conformance Report, and the Statement of Supply to Medicare Australia on: 02 6124 6758 Note: Medicare Your practice will receive a further 12 months of subsidy along with a once off payment of $1000 for completing the Security Awareness and Conformance Report. GETTING A STATEMENT OF SUPPLY FROM TELSTRA
To gain a statement of supply you will need to complete the below web form: http://www.bigpond.com/internet-plans/broadband/adsl/Health/webform.asp This form will capture your Bigpond Username, BigPond Account number and UIN (Unique Identifying Number) The customer then has to agree to the terms and conditions of the re - contract for a further 12 months of service (DoHA Requirement) and once submitted the back of house team will send them a statement of supply with which to make their claim.
